How does a birth chart describe planetary position against constellation/star position?

Is a birth chart geocentric? Does it indicate RA or Hour Angle (some longitudinal based system) or Azimuthal angles? How are the Houses plotted against the zodiac signs? Are the zodiacal divisions accurate to the actual constellation locations?
